- WellSky/Kinnser EMR documentation workflows – Tech Device & Training provided
o including OASIS Start of Care, Recertification, and Resumption of Care visits
o Hands-On On-Boarding & Competency EMR Training w/ continued Operations Support
o Scheduling Optimization and Care Coordination Nursing Teams for clinical services
- Strong Back Office Support from Therapy Department
o Scheduling Assistance / Coordinators
o QA Reviews/ Support
o Documentation Compliance Education / Reviews
o Orders Processing / Physician Follow-up
Salary & Total Compensation
Base Salary: $90K annually (paid bi-weekly)

- You’ll receive genuine, consistent support, including orientation, proper OASIS document training, and a localized clinical management team
* ROLE EXPECTATIONS
- Provide in-home, hands-on PT care, covering Baytown and immediate surrounding areas & counties (utilizing company car for travel)
- Performance & Documentation of PT evaluations, OASIS assessments, care plans, and visit notes in WellSky EMR on provided device with support needs
- Manage a blended caseload of 30–34 visits/week (adjusted weekly based on acuity and visit type)
- Coordinate with nurses, therapy coordinators, and other rehab disciplines
BLENDED VISIT MODEL – Quality vs Volume
Productivity accounts for the complexity of visit types. Time-consuming visits like Start of Care or Recerts will reduce total visit count expectations so you can focus on quality.
THERAPIST QUALIFICATIONS
- Texas-licensed Physical Therapist
- Valid driver’s license and clean driving record
- Strong documentation skills (or willingness to learn with full support)
- Home health experience preferred but not required (If not exp, we will train/educate for success)
