Skilled Nursing Facility | Phoenix, AZ
Compensation: $55–$60 per hour (based on experience)
Schedule: Full-Time | Monday–Friday
– Optional alternative schedules: Tuesday–Saturday or Sunday–Thursday

About the Opportunity
Ahwatukee Post Acute is growing — and we are seeking a dedicated Physical Therapist to join our in-house therapy team! Our 192 bed facility is currently undergoing a major renovation, including construction of a state-of-the-art 44 bed behavioral and dementia care unit, enhancing our ability to provide specialized services to the Phoenix and Ahwatukee communities.
We proudly maintain a CMS 5-Star Quality Measure rating and foster a supportive, collaborative clinical environment focused on patient-centered success.
What You’ll Do

Perform skilled PT evaluations and provide evidence-based treatment

Develop individualized care plans to increase function, safety, and mobility

Document patient progress using Net Health

Collaborate closely with nursing and IDT using PointClickCare
-
Educate patients, families, and caregivers on therapy strategies
You’ll Work With:
Qualifications

Arizona PT license (or eligible prior to start date)

Strong communication and teamwork skills

A passion for working with the geriatric and memory care population
