The Registered Dietitian Nutritionist works directly with patients to assess dietary needs, establish short and long-term health goals, educate about health and nutrition and develop effective nutrition plans.
We are currently seeking a Registered Dietician Nutritionist who can provide 2-4 clinical sessions per month. As demand grows there may be opportunity to expand this role!
YOUR RESPONSIBILITIES-
Provide culturally sensitive, patient-centered care of the highest quality to patients of all backgrounds and income levels.

Assess nutritional and health needs of patients of all ages by reviewing past medical history, obtaining diet history, completing nutritional assessments, and determining appropriate care.

Develop individualized nutrition plans that are sensitive to the cultural and economic needs of our patients.

Provide 1:1 nutrition counseling on healthy eating, chronic disease prevention and dietary issues.

Monitor, evaluate and document outcomes of nutrition assessments, plans and progress.

Maintain records of nutritional care and document notes on all patient visits in EHR system.

Collaborate to create culturally sensitive educational material about healthy eating habits and chronic disease prevention.

Facilitate group education sessions on nutrition topics relevant to our patients.

Actively participate in departmental and/or interdisciplinary meetings, task forces, and projects such as quality improvement initiatives.

Collaborate with Community Health and other departments to identify and obtain dietary and nutrition resources for patients.

Serve as a resource person to other members of the CRCH team around the dietary needs of patients and provide support to other members of the clinical and administrative team in an accessible, open manner.

Communicate effectively and clearly with members of the care team regarding patient status and needs.

Work collaboratively with other Health Center staff in meeting JCAHO and other regulatory and/or funding requirements.

Perform other nutrition-related duties as required by the Medical Director or designee.
YOUR QUALIFICATIONS, COMPETENCIES, TRAITS-
Bachelor's degree required if graduation prior to 2024. Master's degree required if graduation 2024 and thereafter.

Massachusetts Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certification.

1-3 years of clinical nutrition experience, preferably in a community health setting.

Bilingual (Spanish/English, Portuguese/English) is strongly preferred.

Certification in BLS required.

Strong knowledge of evidence-based nutrition guidelines and preventative health practices.

Must have knowledge base of nutritional needs of patients with obesity, diabetes, hypertension, heart disease and other medical conditions benefiting from dietetic and nutritional care.

Experience supporting breastfeeding and lactating individuals, including providing lactation education, guidance, and referrals preferred.

Knowledge of evidence-based lactation and infant feeding practices preferred.

Experience facilitating group education or workshops preferred.

Willingness to educate and mentor clinical support staff as needed.

Must have basic computer skills.

Must have a willingness to work flexible hours to meet the organization's needs/demands.

Must be able to travel to either Charles River Community Health site (Brighton and Waltham) as needed.

Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
