The Case Manager provides comprehensive case management services to residents in a residential treatment setting by coordinating individualized care plans, supporting treatment goals, connecting residents with community resources, and collaborating with clinical, medical, educational, and support teams to promote successful treatment outcomes and transition planning.
The Case Manager serves as a primary advocate for residents, ensuring continuity of care, accurate documentation, and alignment between treatment services and each resident’s recovery, behavioral health, educational, and independent living goals.
Essential Duties and ResponsibilitiesResident Assessment and Case Planning
- Conduct comprehensive assessments to identify resident needs, strengths, barriers, risks, and available support systems.
- Participate in the development, implementation, and review of individualized service plans (ISP), treatment plans, and discharge plans.
- Monitor resident progress toward identified goals and update plans based on changing needs.
- Advocate for resident-centered care while maintaining professional boundaries and confidentiality.
Care Coordination
- Coordinate services between residents, therapists, psychiatrists, medical providers, schools, families, probation officers, and community agencies.
- Facilitate communication among multidisciplinary treatment teams.
- Ensure residents receive appropriate clinical, educational, vocational, medical, and social support services.
- Assist with referrals and linkage to community resources, housing support, healthcare providers, and aftercare programs.
Resident Support and Advocacy
- Build supportive professional relationships with residents while promoting accountability, independence, and personal growth.
- Provide coaching and support related to coping skills, life skills, decision-making, and community reintegration.
- Help residents identify strengths and develop strategies to overcome personal, social, and behavioral barriers.
- Support crisis prevention and intervention efforts in collaboration with clinical staff.
Treatment Team Collaboration
- Attend treatment team meetings, staffing reviews, and case conferences.
- Provide updates regarding resident progress, challenges, and service needs.
- Collaborate with therapists, counselors, direct-care staff, nurses, and administrators.
- Support implementation of evidence-based treatment approaches and organizational policies.
Documentation and Compliance
- Maintain accurate, timely, and confidential case records according to regulatory and organizational requirements.
- Document case management activities, resident contacts, referrals, progress notes, and service coordination activities.
- Ensure documentation meets applicable standards, including licensing, accreditation, HIPAA, and state regulatory requirements.
- Maintain knowledge of agency policies, safety procedures, and reporting requirements.
Discharge and Transition Planning
- Begin discharge planning upon admission and continuously monitor readiness for transition.
- Coordinate aftercare services, community resources, appointments, and support systems.
- Assist residents and families in developing sustainable transition plans.
- Follow up with external providers to promote continuity of care.
Required Qualifications
- Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Behavioral Health, or related field preferred.
- Experience providing case management, residential services, behavioral health, or social services support.
- Knowledge of trauma-informed care, behavioral health principles, and recovery-oriented practices.
- Strong communication, documentation, and organizational skills.
- Ability to work collaboratively within a multidisciplinary treatment environment.
Preferred Qualifications
- Experience working in a residential treatment center, psychiatric facility, substance use treatment program, or youth services environment.
- Experience with electronic health records (EHR) and case management documentation systems.
- Knowledge of Medicaid, behavioral health services, community resources, and discharge planning.
- CPR/First Aid certification preferred.
- Relevant state credentialing or certification (e.g., QMHP, Case Management Certification, Social Work credentials) preferred depending on jurisdiction.
