We are seeking a compassionate and dedicated home healthcare nurse to provide high-quality care to patients in the comfort of their homes. As a home healthcare nurse, you will play a vital role in improving the health and well-being of our patients, assisting with medical needs, and ensuring that families receive the best care possible. This is a full time, benefitted opportunity that works Monday through Friday with occasional weekend coverage.
Key Responsibilities:
- Administering prescribed medications and treatments according to patient care plans.
- Monitoring vital signs, including blood pressure, heart rate, and respiratory function.
- Assessing patient needs and communicating updates to physicians and family members.
- Providing wound care, catheter care, and other specialized nursing procedures as needed.
- Educating patients and families on care techniques, medication management, and healthy lifestyle choices.
- Documenting patient progress and care in medical records.
- Coordinating with interdisciplinary healthcare teams, including physical therapists and social workers.

Requirements:
- Graduate of a state-licensed basic nursing program

Preparation in Community Staff Nursing through a National League for Nursing approved program

With at least one (1) year professional nursing experience under appropriate nursing supervision; or

A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position

Possession of a current State of New Hampshire license to practice as a licensed practical nurse

Maintain current cardiopulmonary resuscitation (CPR) certification

Must be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS' policies and procedures

Sensitivity in human relationships and able to cope with family emotional stress and tolerant of individual lifestyles

Skill in using psychological principles in both one on one and group situations
