McGregor PACE (Program of All-Inclusive Care for the Elderly) is a community-based healthcare program that provides comprehensive medical and supportive services to older adults as an alternative to nursing home placement. Our goal is to help participants remain safe and independent in their homes while receiving coordinated care through our Adult Day Health Center and interdisciplinary team.
We are seeking a compassionate and experienced Case Manager Nurse to join our team. This position is responsible for coordinating participant care, managing clinical needs, and collaborating with the interdisciplinary team to ensure high-quality, participant-centered care.

Position Summary
The Care Manager Nurse serves as the primary nursing coordinator for an assigned caseload of PACE participants. This role provides ongoing clinical assessment, care coordination, participant advocacy, and case management to support participants in achieving their highest level of independence and quality of life.
Essential Responsibilities
- Manage and coordinate care for an assigned caseload of PACE participants.
- Complete nursing assessments and update individualized care plans.
- Coordinate services across the interdisciplinary team and community providers.
- Monitor participants' health status through home visits, clinic visits, and telephone follow-up.
- Provide participants and caregiver education.
- Coordinate transitions of care after hospitalizations or emergency visits.
- Communicate significant changes in condition to the Primary Care Provider and IDT.
- Ensure timely and accurate documentation in the electronic health record.
- Participate in quality improvement and regulatory compliance activities.
- Provide skilled nursing care, including wound care, medication management, catheter care, injections, ostomy care, and other treatments within the scope of practice and organizational policies
- Perform other duties as assigned.
Minimum Qualifications
- Current Ohio Registered Nurse (RN) license.
- Current CPR/BLS certification.
- Valid driver's license and reliable transportation.
- Excellent assessment, communication, and organizational skills.
- Ability to work independently and collaboratively with an interdisciplinary team.
Preferred Qualifications
Minimum of one year of experience working with older adults, home health, geriatrics, case management, or long-term care.
Experience coordinating complex medical and social needs.
- Knowledge of Medicare, Medicaid, and community-based services is a plus.
Pay: $35.00 - $38.00 per hour
