Job Summary:
We are seeking a compassionate and dedicated Hospice Registered Nurse to provide high-quality, patient-centered care to individuals facing terminal illnesses and their families. The RN will be responsible for assessing patient needs, developing care plans, administering medications, and offering emotional and spiritual support to patients and their loved ones.
Responsibilities:
- Conduct comprehensive patient assessments to determine medical, physical, emotional, and spiritual needs.
- Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.
- Administer medications, treatments, and procedures as prescribed, ensuring patient comfort and safety.
- Monitor patient symptoms and response to treatment, adjusting care plans as necessary.
- Provide education and support to patients and families regarding disease progression, symptom management, and end-of-life care options.
- Coordinate with physicians, social workers, chaplains, and other team members to ensure holistic care.
- Facilitate family meetings to discuss care goals, address concerns, and provide resources.
- Document patient care activities and maintain accurate medical records in compliance with regulations.
- Participate in on-call rotations to provide after-hours support as needed.
- Support bereavement services for families post-passing.
Skills:
- Current Registered Nurse (RN) license in [State].
- Bachelor’s degree in Nursing (BSN) preferred; Associate's degree with relevant experience considered.
- Previous hospice or palliative care experience is a plus.
- Strong assessment and clinical skills with a focus on patient-centered care.
- Excellent communication and interpersonal skills to interact effectively with patients, families, and team members.
- Compassionate, empathetic, and able to work in emotionally charged environments.
- Ability to work independently and as part of a multidisciplinary team.
- Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) preferred.
