My Home Doctor (MHD) is a physician-led, home-based primary and chronic care practice. Since 2005 we've brought care directly to frail, medically complex, and chronically ill patients ‚the people who need a clinician most and can least easily get to one. We're looking for an experienced **NP or PA** in the **Pensacola area** to join our team on a **PRN basis**, visiting patients where they live.
This is in-home visits only. You'll need to live in or be willing to travel to the posted service area on your scheduled workdays.
The role at a glance
- **Location:** Gainesville, FL and surrounding county (in-home, on the road)
- **Schedule:** PRN ‚you choose your availability; flexible workdays
- **Type: PRN / 1099 Contract
- **Compensation:** $120 per completed visit + $20 distance rate ‚typical full day runs **$1,040**
- **Population:** Adult and geriatric patients with complex, chronic, and acute needs
What you'll do
- Provide primary, acute, and chronic care in patients' homes ‚as the patient's primary provider or alongside their PCP
- Diagnose and manage conditions like hypertension, diabetes, CHF, COPD, infections, and post-discharge transitions
- Educate patients and families on their conditions, medications, and care plans
- Document in our EMR (eClinicalWorks) with support from our care coordination team
- Collaborate with our supervising physicians and multidisciplinary team
What you'll need
- Active **Florida NP (APRN) or PA license** (DO/MD also welcome to apply)
- National board certification (AANP/ANCC for NPs; NCCPA for PAs)
- **2+ years** of clinical experience; primary care, geriatrics, or home-based experience a strong plus
- Current **BLS** certification
- Reliable transportation and comfort driving to patient homes
- Strong independent clinical judgment and a genuinely patient-centered approach
