Job Title: Speech Language Pathologist (Home Health)
We are seeking a compassionate and dedicated Speech Language Pathologist (SLP) to provide speech therapy services to patients in a home health setting. The SLP will assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders while developing individualized treatment plans to improve patients' communication, swallowing function, and overall quality of life.
Master's Degree in Speech-Language Pathology or equivalent
Active Speech-Language Pathologist (SLP) license in the state of practice
Current BLS/CPR Certification
ASHA Certificate of Clinical Competence (CCC) preferred
Valid Driver's License and reliable transportation
Strong communication, documentation, and patient education skills
Home health or clinical experience preferred
Evaluate patients with speech, language, cognitive, and swallowing disorders.
Develop and implement individualized treatment plans based on patient needs.
Provide speech therapy interventions to improve communication and swallowing abilities.
Monitor patient progress and modify treatment plans as appropriate.
Educate patients, families, and caregivers on therapy techniques and home exercise programs.
Maintain accurate, timely, and compliant clinical documentation.
Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
Ensure compliance with all applicable healthcare regulations, safety standards, and organizational policies.
