Licensed Practical Nurse (LPN) - Care Coordinator
Location: Berks County and surrounding areas, Pennsylvania

Salary: $65,000 - $68,000 annually, based on experience and qualifications
Position Overview
A growing human services organization is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) - Care Coordinator to support individuals receiving residential behavioral health and intellectual/developmental disability (IDD) services throughout Berks County and surrounding communities.
This full-time, field-based position combines nursing services, healthcare coordination, staff training, and regulatory compliance oversight. The ideal candidate will be comfortable traveling between residential homes, collaborating with interdisciplinary teams, and serving as a clinical resource for both individuals served and direct support professionals.

Qualifications
- Active Licensed Practical Nurse (LPN) license in Pennsylvania
- Minimum of two years of nursing experience in residential services, community-based healthcare, behavioral health, IDD services, or a related setting
- Strong clinical assessment and problem-solving skills
- Experience providing training and coaching to direct care staff
- Knowledge of Intellectual and Developmental Disabilities (IDD) and Behavioral Health services preferred
- Familiarity with Pennsylvania DHS and OMHSAS regulations preferred
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and manage multiple priorities
Key Responsibilities
- Provide nursing services in accordance with Pennsylvania Nurse Practice Act requirements
- Monitor health, medical, psychiatric, nutritional, hygiene, and medication-related needs
- Review medication administration records and address medication concerns
- Assist with physician and specialist appointments
- Support complex medical appointments and provide follow-up guidance
- Educate individuals and staff regarding diagnoses, treatments, and healthcare procedures
- Conduct routine site visits to residential programs
- Serve as a liaison between residential teams and healthcare providers
- Provide direct nursing interventions, including injections and wound care, as needed
- Assist with healthcare policy and protocol development
- Coordinate healthcare services for new and current individuals receiving services
- Support discharge planning and post-hospitalization training
- Maintain accurate documentation
- Respond to medical concerns and emergencies as needed
- Serve as a clinical resource to residential teams and interdisciplinary staff
Requirements
- Valid Pennsylvania Driver's License and reliable transportation
- Ability to travel regularly throughout assigned service areas
- Successful completion of all required background checks, including Criminal History and FBI clearances
- Ability to pass pre-employment physical and drug screening requirements
- Ability to complete and maintain all required trainings and certifications
