Responsibilities:
- Provide therapy services to patients in a skilled nursing facility
- Conduct patient assessments, plan of cares
- Administer therapy sessions to improve patients' mobility and maximize independence
- Collaborate with healthcare professionals, including doctors, nurses, and other therapists to coordinate patient care
- Educate patients and their families on therapy techniques and exercises for continued progress
- Maintain accurate and up-to-date patient records
Skills:
- Strong knowledge of therapy techniques and practices
- Ability to assess patients abilities and develop individualized treatment plans
- Understanding of human anatomy and physiology related to physical mobility
- Familiarity with medical terminology used in a skilled nursing setting
- Excellent communication skills to effectively interact with patients, families, and healthcare professionals
- Ability to work in a post acute environment with compassion and empathy for patients' needs.
