Position Summary
The SCN Screener & Care Navigator is responsible for conducting standardized social needs screenings, engaging members to identify unmet social determinants of health (SDOH), and facilitating referrals and linkages to appropriate community-based services within the Social Care Network (SCN). This role supports members in navigating services related to food insecurity, housing instability, transportation, utilities, and other social needs, while ensuring accurate documentation and compliance with NY Medicaid and SCN requirements.
Key Responsibilities
Screening & Assessment
- Conduct standardized SCN social needs screenings (e.g., food, housing, utilities, transportation, interpersonal safety, etc.)
- Engage members telephonically and/or in person using trauma-informed and culturally competent approaches
- Identify eligible SCN services and determine appropriate next steps based on screening results
- Explain available social care services and member rights in clear, understandable language
Care Navigation & Referral Coordination
- Initiate and coordinate referrals to approved SCN vendors and community-based organizations
- Support members in completing referrals and overcoming barriers to service access
- Track referral outcomes and follow up to ensure services are initiated and completed
- Coordinate with care managers, health plans, and SCN partners as needed
Required Qualifications
- High school diploma or equivalent (Associate’s or Bachelor’s degree preferred in human services, social work, public health, or related field)
- Experience conducting screenings, outreach, or care coordination in healthcare, social services, or community-based settings
- Strong verbal and written communication skills
- Ability to engage diverse populations with sensitivity and professionalism
- Proficiency with electronic documentation systems and basic computer applications
Starting pay is $21/ Hour
Pay: $21.00 per hour
