Planet Home Health Care, Inc. is currently hiring experienced Licensed Vocational Nurses (LVNs) for per diem home health visits.
We are looking for reliable, professional, and compassionate LVNs who are comfortable working independently in the field and providing quality patient care in the home setting. Experience working with Veterans, VA patients, TriWest/VA CCN patients, or military-connected patients is strongly preferred.
Position Responsibilities
- Provide skilled nursing visits in the patient’s home under the supervision of an RN
- Monitor patient condition and report changes appropriately
- Provide medication management and patient/caregiver education
- Perform wound care, injections, disease management, and other skilled nursing services as ordered and within LVN scope of practice
- Follow the established plan of care
- Communicate with the supervising RN, physicians, case managers, and agency office as needed
- Complete accurate and timely documentation
- Maintain professional communication with patients, families, and the care team
Requirements
- Current California LVN license
- Minimum 1 year of nursing experience preferred
- Home health experience strongly preferred
- Experience with Veterans / VA / TriWest patients is a plus
- Wound care experience
- Reliable transportation
- Valid driver’s license and auto insurance
- Ability to travel to patient homes within assigned service areas
- Strong communication and documentation skills
- Must be dependable, professional, and able to manage visits independently

Pay: $35.00 - $50.00 per hour
