SAJE Strategic Solutions is seeking a highly skilled and compassionate Physical Therapist (PT) to join a dynamic rehabilitation team on a Full-Time basis inside a premier Skilled Nursing Facility (SNF) in the San Gabriel Valley, CA. This position offers a competitive hourly rate of $55.00 – $62.00 per hour to help residents regain functional independence, restore mobility, and improve their overall quality of life.
Key Responsibilities
- Evaluations & Assessments: Conduct comprehensive initial evaluations of residents to determine physical conditions, functional limitations, and rehabilitation potential.
- Plan of Care: Develop and implement individualized, evidence-based treatment plans focused on gait training, strength building, and pain management.
- Supervision: Provide clinical supervision to Physical Therapist Assistants (PTAs) and Rehab Aides to ensure the highest standards of care are met.
- Interdisciplinary Collaboration: Participate in care conferences and work closely with nursing, occupational therapy, and speech-language pathology teams to coordinate effective discharge planning.
- Documentation: Maintain timely and accurate clinical documentation (including evaluations, daily progress notes, and discharge summaries) in strict compliance with Medicare and state regulations.
- Patient Advocacy: Educate residents and their family members on safety protocols, proper mobility techniques, and personalized home exercise programs.
Qualifications & Requirements
- Education: Graduate of an accredited Physical Therapy program (DPT preferred).
- Licensure: Current, valid California Physical Therapist License in good standing.
- Certification: Basic Life Support (BLS) certification required.
- Experience: Previous experience working within a Skilled Nursing Facility (SNF) or geriatric care setting is highly preferred.
- Industry Knowledge: Proficiency with EMR systems and a solid understanding of PDPM and long-term care regulations.
- Physical Demands: Ability to safely assist with patient transfers and lift or move up to 50 lbs, alongside standing, walking, and bending for the duration of the shift.
