This role is perfect for someone who is compassionate, organized, and energized by building relationships. You’ll work closely with residents, families, and an interdisciplinary care team to help ensure every resident feels heard, supported, and cared for.
What You'll Do:
- Help support residents’ social, emotional, and spiritual well-being
- Assist with admissions and discharges, helping residents and families navigate transitions smoothly
- Provide emotional support and crisis assistance when it matters most
- Act as a connector between residents, families, care teams, and community resources
- Participate in care planning meetings and collaborate with a supportive interdisciplinary team
- Assist with documentation, including social services sections of required records
- Build and maintain relationships with local and state community agencies

What You'll Need:
- High school diploma or equivalent preferred
- Completion of (or willingness to complete) Nebraska Health Care Association Social Service Designee training
- A warm, compassionate personality and strong people skills
- Ability to communicate clearly, maintain confidentiality, and stay organized
- Basic computer skills and comfort with documentation
- A genuine interest in working with older adults
