20 hours/week, 5 days a week. Starting at $37.00/hour. Candidates must old an active CPR certification.
POSITION SUMMARY:
The Adult Day Services (ADS) Nurse supports the health, safety, and wellbeing of participants in the Adult Day Services setting. This role includes monitoring participant health status, providing nursing care, administering medications (as allowed by licensure), maintaining documentation, and collaborating with interdisciplinary staff to ensure highquality, personcentered care.
ESSENTIAL JOB FUNCTIONS
- Provides direct nursing care to adult day service participants, including routine health assessments and monitoring.
- Administers medications and treatments consistent with Illinois state nursing regulations and facility policies.
- Follows medication procedures, including medication storage, labeling, administration, documentation, medication error reporting, and communication with families/caregivers and prescribers as appropriate.
- Documents health assessments, medication administration, and changes in participant condition accurately and promptly.
- Participates in reviewing and updating personcentered care plans. (Monthly RN review required if position is filled by an LPN under RN supervision).
- Responds to health concerns and emergencies, providing first aid and coordinating with emergency services when necessary. Completes or supports incident reports and health-related follow-up after falls, injuries, medication concerns, changes in condition, or emergency events.
- Communicates regularly with families, caregivers, and interdisciplinary staff.
- Maintains compliance with all Illinois Department on Aging and Adult Day Service regulations.
- Participates in quality assurance, safety protocols, and required staff meetings.
- Supports training related to health, infection control, and medication procedures for other staff as needed.
- Provides personal care (toileting, dressing, mobility support, hygiene, etc.).
- Actively participates in activities – both individual and group – to fully engage the participants.
- Assists with meal service, including setup, serving, and cueing; monitors participants during meals for safety and nutrition needs.
EDUCATION, CERTIFICATES, LICENSES AND EXPERIENCE
- Must be a Registered Nurse (RN) licensed in Illinois OR a Licensed Practical Nurse (LPN) licensed in Illinois under the supervision of an RN (Supervising RN may be contractual but must meet with LPN at least monthly to review care plans and medication records and be available for guidance).
- Must be CPR and First Aid Certified
PREFERRED QUALIFICATIONS
- Two years of experience working with older adults, adults with disabilities, or in communitybased care settings.
- Prior adult day service or longterm care experience.
