Join our dynamic healthcare team as a Travel Patient Care Coordinator, where your expertise will drive exceptional patient experiences across diverse clinical settings. In this vital role, you will coordinate patient care journeys, ensuring seamless communication between healthcare providers, patients, and families. This role requires regular travel between our clinic locations in Gilbert, Peoria, Prescott, Tucson, and West Phoenix, Arizona to support patient care and clinic operations.

Duties
- Collaborate closely with healthcare professionals across hospital, outpatient, and post-acute settings to coordinate patient care plans.
- Conduct comprehensive patient assessments, including vital signs, medical history review, and physiological evaluations to inform clinical decision-making.
- Manage patient intake processes and discharge planning to ensure smooth transitions between care levels.
- Utilize Electronic Health Records (EHR) systems to document medical records accurately and maintain HIPAA compliance.
- Assist in diagnostic evaluation and case management for diverse populations including pediatrics, geriatrics, behavioral health, and infectious disease cases.
- Support utilization management by evaluating medical necessity and coordinating with insurance providers for managed care processes.
- Provide exceptional patient service by communicating effectively with patients and their families, addressing concerns, and explaining treatment options clearly.
Skills
- Extensive critical care experience including ICU, emergency room, Level I & II trauma centers, and acute care environments.
- Proven hospital medicine expertise with familiarity in medical-surgical units, OR procedures, and post-acute care settings such as hospice or home health.
- Strong knowledge of health regulation policies including HIPAA standards and medical terminology.
- Skilled in managing patients as a nurse case manager or through clinical case management roles with a focus on discharge planning and inpatient/outpatient coordination.
- Proficient in EMR/EHR management systems with the ability to document accurately and efficiently.
- Excellent client communication skills with the ability to collaborate effectively with multidisciplinary healthcare teams.
- Experience in managing complex cases involving infectious diseases, behavioral health issues, geriatrics, pediatrics, or managed care programs.
