Job Description: Provide case management, advocacy and monitoring services to adults who have disabilities and/or mental health challenges, as well as to children and families. Provide resources and outreach to help people improve their quality of life and ability to successfully function at home, school/work, and in the community. Home visits take place throughout New Jersey. Position is 65% field/ 35% in Somerville, NJ office. Qualifications: BSW/BA/BS in human services, social work, or related area plus 2 years of case management experience working with people who have disabilities and/or mental health challenges. Other social services or case work experience will be considered, and Spanish language proficiency a plus. Ideal candidate will have knowledge of NJ benefit systems, including Medicaid, Medicare, SSI, SSDI, SNAP, and how to access benefits and other public resources. Proven case management skills and comfort working autonomously, and in a team-based setting. Benefits related to health coverage are non-contributory and a valid driver’s license with an excellent driving record are required.
