Client Care Advocate Job Description
Position Summary
The Client Care Advocate serves as a vital bridge between clients, families, clinical teams, and aftercare providers throughout the treatment process. This role focuses on building meaningful relationships with clients, supporting emotional safety and trust, coordinating individualized aftercare planning, and helping clients successfully transition into the next phase of recovery care.
The ideal candidate is compassionate, organized, emotionally intelligent, and passionate about helping individuals suffering from substance use and mental health disorders find long-term recovery solutions.
Essential Duties & Responsibilities
- Build rapport and establish trusting relationships with clients throughout treatment.
- Meet individually with clients to assess readiness, motivation, and aftercare needs.
- Coordinate discharge planning and continuum-of-care recommendations alongside clinical teams and case managers.
- Educate clients and families on the importance of continued care, including PHP, IOP, sober living, therapy, and community support.
- Collaborate with treatment centers, sober living homes, outpatient providers, and community resources to secure appropriate placements.
- Conduct tours, introductions, and warm handoffs to aftercare programs when appropriate.
- Participate in clinical meetings and communicate updates regarding client progress and discharge planning.
- Advocate for client needs while maintaining professional boundaries and ethical standards.
- Maintain accurate documentation, communication logs, and referral tracking.
- Assist with alumni engagement, recovery community events, and ongoing client support initiatives.
- Support crisis management and provide emotional support while directing clinical concerns to licensed staff when necessary.
- Represent the organization professionally with clients, families, referral partners, and staff.
Qualifications
- Experience working in behavioral healthcare, substance abuse treatment, mental health, or recovery support services preferred.
- Strong interpersonal and communication skills.
- Ability to build trust with clients and families in high-emotion situations.
- Excellent organization, multitasking, and follow-through abilities.
- Knowledge of levels of care including detox, residential, PHP, IOP, sober living, and community recovery resources.
- Proficient in documentation and professional communication.
- Ability to work collaboratively within a multidisciplinary team environment.
- Personal recovery experience is welcomed but not required.
Preferred Qualifications
- CADT, RADT, CCAPP, or related certification preferred.
- Previous experience in discharge planning, alumni coordination, admissions, case management, or client advocacy.
- Familiarity with insurance processes and treatment placement coordination.

Work Environment
This position operates within detox, residential, and/or outpatient behavioral healthcare settings and may involve communication with clients in emotional or crisis situations. Flexibility, professionalism, and teamwork are essential to success in this role.
Pay: $23.00 - $25.00 per hour
