Position Overview
The Lead RN will be responsible for developing and leading the Clinical Assistant team at ApogeeCare. The Clinical Assistant is essential in supporting ApogeeCare’s telehealth services in ED settings, providing direct assistance to remote clinicians during assessments of older adults. This role involves program development and oversight, training, supervision, patient care, telehealth coordination, and administrative tasks to ensure a comfortable and efficient experience. Additionally, the clinical assistant will act as ApogeeCare’s representative in the ED, liaising between internal and external teams to coordinate comprehensive patient care. This position requires regular in-person shifts in the ED and reports directly to the Medical Director.
KEY RESPONSIBILITIES
Leadership and Support:
- Working in partnership with Leadership to establish best practices, operational enhancements, and care delivery standards.
- Supervise and support the clinical assistant team.
- As the team grows, oversee the onboarding and training of team members.
- Split time between direct patient care and leadership. As we scale this role will focus on the managerial aspects of the program.
Telehealth Coordination:
- Identify and prepare older adults (65+) eligible for ApogeeCare telehealth services.
- Introduce ApogeeCare’s telehealth services to patients and families and obtain informed consent.
- Conduct basic geriatric screenings under the direction of remote clinicians, including cognitive, mobility, and functional assessments.
Patient Care Support:
- Assess and document assessment within EMR. Appropriate assessment is determined by patient presentation, physician recommendation and partnership with ED and Hospital leadership.
- Perform clinical tasks such as orthostatic blood pressure, visual acuity testing and/or hearing assessment within the scope of an RN.
- Maintain accurate documentation within electronic health records (EHR) systems, such as Epic, and support clinicians by organizing patient documentation.
Technology and EHR Management:
- Proficient with computers and being able to trouble shoot issues as they arise.
- Utilize electronic health records (EHR) efficiently for documentation, patient history, and clinical updates, ensuring all data aligns with organizational standards.
- Set up and manage telehealth audio and video equipment (e.g., camera, tablet, audio equipment), and provide troubleshooting support as needed during telehealth consultations.
- Educate patients on using MyChart or similar patient portals to access their health information, appointment scheduling and telehealth services.
Assessment Tools and Best Practices:
- Apply or develop proficiency in using standardized geriatric screening tools, such as the ISAR, Mini-Cog, and Katz ADL Index, to support accurate patient evaluations among other tools providers use to evaluate patients.
- Collaborate with remote clinicians to implement best practices in geriatric assessments, and follow up on patients identified at higher risk for adverse health outcomes.
Liaison and Partnership Development:
- Act as ApogeeCare’s representative in the ED, fostering strong partnerships with ED staff, physicians, nurses, social workers, and other support departments to coordinate age-friendly, patient-centered care.
- Serve as the main point of contact for ApogeeCare, ensuring seamless communication and collaboration between ApogeeCare’s remote clinicians and the ED team.
- Facilitate connections with external vendors and community support services, including Assisted Living communities, Skilled Nursing Facilities, Adult Protective Services, and other agencies to coordinate post-discharge care and resources for patients.
- Engage regularly with community partners to promote comprehensive support for older adults, especially those at higher risk for adverse outcomes following ED visits.
Required Qualifications
- Registered Nurse (RN) credential with active state licensure.
- At least 3 years of experience, emergency medicine preferred.
- BLS certification required; additional geriatric care training is a plus.
- Strong interpersonal and communication skills, particularly with older adults, and the ability to work effectively in a team environment.
- Proficiency with computers, typing, and managing multiple computer programs at the same time. Willingness to trouble shoot and problem solve computer issues as they arise on their own.
- Proficiency in electronic health record systems (e.g., Epic) and familiarity with telehealth and video communication technology.
- Willingness and ability to learn or expand knowledge of geriatric assessment tools.
