Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Administer medications via IV infusion, injections, and other routes while adhering to aseptic techniques and HIPAA regulations to ensure safety and confidentiality.
- Manage complex wound care procedures such as suturing and ostomy care while educating patients and families on proper techniques.
- Utilize EMR systems like Epic or Cerner to document patient interactions accurately and efficiently, ensuring compliance with medical documentation standards.
- Coordinate discharge planning and case management activities to facilitate smooth transitions from hospital or facility to home care settings.
- Provide specialized treatments including botulinum toxin injections for ophthalmology patients or pain management interventions for chronic conditions.
- Collaborate with multidisciplinary teams across hospital medicine, ICU, trauma centers, or outpatient clinics to deliver comprehensive patient-centered care.
Skills
- Extensive experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth for seamless documentation and communication.
- Proficiency in advanced nursing procedures including IV insertion, catheterization, ventilator management, and laboratory specimen collection/processing.
- Strong knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and diagnostic evaluation techniques.
- Critical care experience in ICU or Level I/II trauma centers with expertise in trauma medicine and acute pain management.
- Skilled in working with diverse populations including pediatrics, geriatrics (including memory care), individuals with developmental disabilities or dementia/dementia care.
- Ability to perform patient assessments involving physiology knowledge, airway management, infection control practices, and laboratory testing.
- Experience in home & community care environments along with familiarity with medical office operations such as triage and health coaching.
