Job Description: Treatment Nurse (Skilled Nursing Facility)
Position Title: Treatment Nurse (LVN)

Position Summary
The Treatment Nurse is responsible for providing specialized nursing care focused on wound management, skin integrity, infection prevention, and physician-ordered treatments for residents in a skilled nursing facility. The Treatment Nurse collaborates with the interdisciplinary team to promote healing, prevent complications, and ensure compliance with federal, state, and facility regulations.
Essential Duties and Responsibilities
- Perform comprehensive skin assessments upon admission, weekly, and as clinically indicated.
- Assess, treat, and monitor wounds, pressure injuries, surgical incisions, burns, and other skin conditions.
- Administer physician-ordered treatments, dressings, medications, and topical therapies according to established standards of nursing practice.
- Develop, implement, and update individualized wound care plans.
- Monitor residents' response to treatments and promptly report changes in condition to the attending physician and nursing leadership.
- Document all assessments, treatments, and resident responses accurately and timely in the electronic medical record (EMR).
- Coordinate wound care rounds with physicians, nurse practitioners, and the interdisciplinary care team.
- Ensure appropriate use of wound care products and treatment supplies.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities related to skin integrity and wound prevention.
- Monitor residents for signs and symptoms of infection and implement infection prevention measures.
- Educate residents, families, and staff regarding wound care, pressure injury prevention, and skin care practices.
- Assist with regulatory surveys and maintain compliance with CMS, California Department of Public Health (CDPH), OSHA, and facility policies.
- Maintain confidentiality of resident information in accordance with HIPAA regulations.
- Perform other nursing duties as assigned.
Qualifications
- Current California Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license in good standing.
- Current CPR/BLS certification.
- Minimum of one (1) year of nursing experience in a skilled nursing facility, rehabilitation, or long-term care preferred.
- Experience in wound care, treatment nursing, or certification in wound management preferred (e.g., WCC, WOCN).
- Knowledge of federal and California long-term care regulations.
- Strong assessment, documentation, organizational, and communication skills.
- Ability to work independently and collaboratively with the interdisciplinary team.
Physical Requirements
- Ability to stand, walk, bend, reach, and lift up to 50 pounds with or without reasonable accommodation.
- Frequent use of hands for treatments and documentation.
- Ability to respond quickly to emergency situations.
- Ability to wear appropriate personal protective equipment (PPE).
Working Conditions
- Skilled nursing and rehabilitation facility environment.
- Exposure to infectious diseases, bodily fluids, and hazardous materials consistent with healthcare settings.
- May require weekends, holidays, and occasional overtime based on facility needs.

Pay: $38.00 - $41.00 per hour
