Senior Wellness Outreach Specialist – Full -Time
REPORTING RELATIONSHIP
Reports to the Senior Wellness Outreach Supervisor & Reports to the Director of Home Care
SUMMARY
The Senior Wellness Outreach Specialist participates in a nonprofit community wellness initiative focused on reducing social isolation among older adults through sustained engagement, targeted outreach, and relationship-based programming. This role is responsible for identifying socially isolated individuals, facilitating meaningful social connections, and ensuring programs are accessible to homebound and underserved populations. This will be done through outreach and proper documentation is required.
RESPONSIBILITIES
- Plan, implement, and manage social isolation prevention programs that promote regular, meaningful social interaction among older adults.
- Identify and engage socially isolated individuals through referrals, community partnerships, outreach, and direct engagement.
- Ensure programs are inclusive of homebound seniors and incorporate in-person or hybrid participation models beyond virtual engagement alone.
- Integrate intergenerational programming opportunities that foster connection between older adults and other age groups.
- Lead targeted outreach to underserved populations, including non-English-speaking and socially marginalized older adults.
- Develop trust-centered, culturally responsive engagement strategies to reduce barriers to participation.
- Implement participant retention strategies that support consistent attendance and long-term engagement.
- Maintain ongoing communication with participants to support sustained social connection and wellness.
- Build and maintain partnerships with healthcare providers, social service agencies, faith-based organizations, and community groups.
- Coordinate communication with families, caregivers, volunteers, and service providers while preserving participant confidentiality.
- Support volunteer recruitment, orientation, and coordination related to program activities.
- Promote program visibility across the service area to encourage participation, partnerships, and volunteer involvement.
- Represent the organization at community events, coalition meetings, and outreach activities.
- Contribute to outreach materials and messaging aligned with the organization’s mission and values.
- Manages own caseload of participants
- Assess baseline social isolation levels and track participant progress throughout program participation.
- Collect and manage program data using evidence-informed or established evaluation tools.
- Ensure secure storage of participant data and adherence to confidentiality and data protection standards.
WORK ENVIRONMENT
- Combination of office, community-based, and fieldwork settings
- Monmouth County Travel Required
- Occasional evening or weekend hours for outreach or events
- Will require light lifting of materials for events

BACKGROUND/EDUCATION/TRAINING/EXPERIENCE
- Associates degree
- Strong interpersonal, organizational, and communication skills.
- Experience with data collection, outcome tracking, or program evaluation.
- Understanding of Volunteer population
- Understanding of the Older Adult population
- Must be computer literate
- Must have valid NJ Drives License
