Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN), where your expertise will make a meaningful difference in patient care and well-being. In this vital role, you will be the AWV coordinator for the clinic to schedule and manage follow up appointments. Your skills in patient assessment, medication administration, and clinical documentation will support our mission to deliver exceptional, personalized care. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care.
- Assist with diagnostic evaluations such as specimen collection, laboratory processing, and medical imaging procedures.
- Manage complex patient needs in a rural health clinic setting.
- Provide specialized care for patients with chronic conditions such as Alzheimer's disease, dementia, or developmental disabilities within memory or long-term care environments.
- Collaborate with multidisciplinary teams to develop and implement individualized discharge plans and case management strategies.
- Ensure compliance with HIPAA regulations, infection control policies, and long-term care regulations while maintaining meticulous medical records and documentation.
Qualifications
- Valid LVN license in good standing with current state certification.
- Demonstrated experience working with diverse populations including geriatrics, pediatrics, or individuals with disabilities.
- Proficiency in EMR/EHR systems such as Epic, Athenahealth, or eClinicalWorks; experience with ICD coding (ICD-9/ICD-10) is highly desirable.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology to support accurate patient assessment and treatment planning.
- Ability to perform basic life support (BLS), CPR, sterile techniques, venipuncture, catheterization, and first aid procedures confidently.
- Excellent communication skills for effective caregiver communication and client interaction in fast-paced environments.
