Licensed Vocational Nurse (LVN) – Service Care Coordinator Job Summary
We are seeking a compassionate and organized Licensed Vocational Nurse (LVN) to join our Care Management team as a Service Care Coordinator. In this role, you will coordinate care for members by conducting comprehensive assessments, developing individualized care plans, educating members and caregivers, and connecting members with appropriate healthcare and community resources. This position requires strong clinical judgment, excellent communication skills, and the ability to manage a caseload while ensuring high-quality, cost-effective care.
Key Responsibilities
- Conduct comprehensive in-home and community-based assessments to evaluate members' medical, behavioral, and social needs.
- Develop, implement, and update individualized service care plans in collaboration with members, caregivers, providers, and interdisciplinary teams.
- Coordinate healthcare services, referrals, and community resources to ensure members receive appropriate care.
- Educate members and their families on available healthcare benefits, services, and treatment plans.
- Perform proactive outreach and ongoing case management to monitor member progress and address barriers to care.
- Collaborate with physicians, healthcare providers, and internal care management teams to improve health outcomes.
- Identify health and safety concerns and facilitate appropriate interventions.
- Maintain accurate, timely, and compliant documentation in accordance with company policies and regulatory requirements.
- Ensure all assessments, care plans, and documentation are completed within required turnaround times.
- Support quality improvement initiatives while promoting member satisfaction and positive healthcare outcomes.
Schedule & Work Environment
- Hybrid field-based position.
- Approximately 75% local travel to member homes and community locations.
- Three days per week dedicated to field visits, completing approximately 2–3 in-person member assessments daily.
- Two days per week focused on documentation, care planning, care coordination, and follow-up activities.
- Requires reliable transportation and the ability to travel within the assigned service area.
Required Qualifications
- Active Licensed Vocational Nurse (LVN) license in the applicable state.
- Experience in care management, case management, home health, long-term care, managed care, or community health.
- Strong knowledge of healthcare coordination, member assessments, and care planning.
- Excellent communication, organizational, and time management skills.
- Ability to work independently while collaborating effectively with multidisciplinary teams.
- Proficiency with electronic medical records (EMR) and documentation systems.
- Valid driver's license, reliable transportation, and willingness to travel extensively.
Preferred Qualifications
- Experience working with Medicaid, Medicare, or long-term services and supports (LTSS) populations.
- Prior experience conducting in-home assessments.
