Position Purpose: The Registered Nurse Case Manager is a central figure in delivering effective, empathetic, and efficient hospice care to patients in their homes. This role demands a combination of clinical expertise, compassionate patient interaction, and detailed case management to ensure that patients receive comprehensive end-of-life care aligned with their needs and preferences.
Key Responsibilities:
- Patient Care Management:
- Assess patient needs through regular visits and coordinate with healthcare professionals to develop and implement tailored care plans.
- Manage the overall provision of hospice services to patients during each duty period.
- Communication and Coordination:
- Serve as the primary point of communication between patients, families, and the interdisciplinary healthcare team.
- Coordinate patient care activities and share information timely and effectively to ensure consistency in care delivery.
- Clinical Practices and Protocols:
- Administer medications and treatments as prescribed and monitor responses.
- Maintain up-to-date patient records and case notes on symptoms, treatments, and progress.
- Education and Support:
- Educate patients and their families on illness progression, symptom management, and end-of-life care.
- Provide emotional support and coping strategies for patients and caregivers.
- Quality Assurance:
- Monitor and ensure compliance with all local, state, and federal regulations regarding hospice and home health care.
- Participate in regular case reviews and team meetings to enhance quality of care.
- Team Collaboration:
- Work collaboratively with other team members, including physicians, social workers, and hospice aides, to facilitate holistic care.
- Mentor and guide team members to improve patient outcomes and foster a supportive work environment.
- Attend IDG regularly.
Qualifications:
- Current Registered Nurse (RN) license in the state of practice.
- Minimum of one year of nursing experience in hospice or palliative care settings preferred.
- Cardiac experience preferred
- Strong clinical skills with the ability to work independently and make critical decisions.
- Excellent communication and interpersonal skills, capable of handling complex family dynamics.
- Knowledge of Medicare, Medicaid, and private insurance processes as they relate to hospice care.
- BSN from an accredited school of nursing preferred
Physical and Environmental Requirements:
- Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
- Must be able to handle exposure to elements such as weather and driving conditions.
Job Type: Full-time
Pay: $82,000.00 per year
