The Case Manager supports Residents within the EAC unit by coordinating care across the interdisciplinary team, ensuring medical necessity, facilitating transitions, connecting Residents to community-based services. This role is essential to the EAC program’s philosophy of recovery-oriented, trauma informed care, community integration and step-down readiness. The Case Manager ensures that Residents have access to appropriate supports during their stay and upon discharge.
QUALIFICATIONS:

BA DEGREE IN SOCIAL WORK OR RELATED FIELD REQUIRED.

Minimum of 2 years in Behavioral Health, preferably inpatient or residential care

Experience with complex case management needs Strong organizational, communication, and interdisciplinary collaboration skills.

Experience in working with community resources and payor groups.
