Deliver skilled nursing care to patients in their homes, guided by an experienced RN supervisor.

Collaborate with interdisciplinary team members for quality, coordinated care.

Observe, report, and document changes in patient status and help implement care plans.

Participate in patient education, medication review, and ongoing care assessments.

Prepare progress notes and ensure all documentation meets regulatory requirements.

Licensed Vocational Nurse (LVN) with current Texas nursing license.

Minimum 1 year LVN experience (home health a plus, but willing to train highly motivated candidates).

Reliable car, valid Texas driver’s license, and current auto insurance.

Current CPR certification, negative TB screen, and Hepatitis profile.

Familiarity with Kinnser/WellSky preferred but not required.

Team player with great communication skills, adaptability, and reliability.

Ability to lift up to 50 pounds and perform regular standing/walking as needed.
