The VNA is seeking a Non-clinical Referral Specialist to work with our Referral Center team. This position will be responsible for accurate referral information from patients, MD's, MD offices, hospitals, nursing homes, etc. and the coordination of referrals for all companies and service lines.
Responsibilities
- Responsible for ensuring the Intake team completes the referral process in an accurate and timely manner.
- Maintaining and performing timely intake processes on referrals including completion of the appropriate section of the HCHB chart when time allows.
- Maintaining an organized system.
- Participating in team meetings, handling correspondence, sharing meeting responsibilities, responding to routine requests, and assisting and promoting the collaboration of professional learning and professional development throughout the division and agency.
- Requests details from referral sources and patients when appropriate for obtaining complete documentation.
- If necessary, assignment of referrals to appropriate intake specialists for completion.
- Proficient in using computers for data entry; have knowledge of Microsoft Excel, Word, Adobe, preferred HCHB/Forcura experience with working with data base management.
- Demonstrate patience and a pleasant demeanor. Possess a high level of problem-solving skills. In addition, flexibility and the ability to be detail oriented is required.
- Maintain good professional relationships with all internal and external customers and provide accurate information for all processes.
- Maintain optimal quality standards for all processes and ensure compliance to productivity schedule and monitor all activity.
- Provide orientation of new staff to referral center when assigned. Teach and train as needed.
Qualifications:
Associate degree (A. A.) or equivalent from two-year college or technical school. Prior home care experience preferred. Must possess computer skills - knowledge of Microsoft Word, Excel, Meditech and PTCT preferred.
