At HopeWest, we believe exceptional care begins with exceptional people. We are looking for a compassionate, skilled Licensed Practical Nurse (LPN) to join our growing PACE (Program of All-Inclusive Care for the Elderly) team.
PACE provides comprehensive, coordinated healthcare and supportive services that allow older adults with complex medical needs to remain living independently in their homes and communities for as long as possible. As an LPN, you will play an important role in delivering hands-on care, supporting participants and families, and collaborating with an interdisciplinary team committed to providing person-centered care.
As a PACE LPN, you will provide direct nursing care and clinical support to participants in their homes. Responsibilities may include:
Provide direct nursing care to PACE participants in accordance with individualized care plans and provider orders.
Monitor participant health status and promptly communicate changes in condition to the appropriate clinical team members.
Administer medications and treatments within the scope of LPN practice.
Collaborate closely with RNs, providers, CNAs, therapists, social workers, and other members of the interdisciplinary team.
Support participant education and encourage independence, safety, and overall well-being.
Help ensure a welcoming, compassionate, and participant-centered experience.
Maintain accurate and timely clinical documentation.
Follow all applicable regulations, policies, procedures, and standards of care.

Current and valid Licensed Practical Nurse (LPN) license in the State of Colorado, maintained throughout employment.
Graduate of an accredited practical nursing program.
Current and valid Colorado driver's license, adequate automobile insurance and unlimited access to reliable transportation.
Previous nursing experience preferred; experience working with older adults or individuals with complex medical needs is a plus.
Strong clinical judgment and the ability to recognize and communicate changes in participant condition.
Excellent communication and interpersonal skills.

Competitive pay structure $29.82-$33.00 per hour depending on experience.
