The Social Work Case Manager works closely with the interdisciplinary team to ensure residents receive comprehensive, person-centered support. This role focuses on care coordination, documentation, resident engagement, and family communication.
Assist with resident assessments and care plan development
Support case management documentation and compliance
Coordinate appointments and community resources
Provide emotional support and advocacy for residents
Communicate with families regarding care updates and services
Assist with discharge planning and transitions
Maintain accurate records in accordance with state regulations
Participate in interdisciplinary team meetings
