Compassion Health Care is seeking a dedicated and adaptable Clinical Care Coordinator (LPN, CMA) – Floater to support our clinical teams across multiple service lines, including Primary Care, Psychiatry, Urgent Care, and our School-Based Health Center. This position will support these service lines at both Caswell Family Medical Center and James Austin Health Center, and our Mobile School-Based Health Center (RV) as needed.
The primary responsibilities of the Float Clinical Care Coordinator include assisting providers with patient care needs, supporting referral and refill workflows, managing clinical documentation, and providing coverage when primary Clinical Care Coordinators are out. As a Clinical Care Coordinator at Compassion Health Care, you will be part of a collaborative healthcare team within a Patient-Centered Medical Home model, serving patients of all ages. This role utilizes the nursing process—including assessment, planning, implementation, evaluation, and documentation—to support patient care and education. The coordinator anticipates provider needs, promotes efficient clinic flow, and collaborates with other members of the healthcare team to support comprehensive care. The role follows all guidelines within the North Carolina Nurse Practice Act and CHC’s policies and procedures. Cross-training in various departments may be required to support organizational needs.
Job Responsibilities
- Utilize nursing knowledge, clinical judgment, and critical thinking to complete timely and accurate assessments.
- Assess individual patient needs through clinical observation, medical record review, and patient/family interaction.
- Consider cultural diversity and lifestyle factors in patient care planning.
- Implement provider orders effectively and safely.
- Administer medications as ordered and document appropriately within the EMR.
- Perform and assist with medical procedures using approved techniques and safety standards.
- Ensure learning needs of patients and families are identified and addressed.
- Work collaboratively with providers and staff to support patient care needs.
- Perform referral tracking and follow-up for completed and pending referrals.
- Capture and document Chronic Care Management (CCM) time as appropriate.
- Support coverage needs across all clinics and the Mobile Unit; must be willing to travel between sites and operate the mobile RV (training provided).
- Maintain a positive, professional, and cooperative working relationship with all teams.
