CareerNation is seeking experienced and compassionate Licensed Vocational Nurses (LVNs) for an ongoing private-duty home care assignment in New Braunfels, TX. Both day and overnight coverage are available.
This is a 1:1 private-duty case supporting an adult patient in her home. The patient requires skilled nursing support, including blood glucose monitoring and insulin administration. She is also experiencing early signs of cognitive decline/dementia and may have periods of confusion, disorientation, agitation, and aggressive behavior.
We are looking for nurses who are calm, patient, confident, and comfortable providing care to a patient experiencing behavioral and cognitive changes. The right nurse should be able to redirect and de-escalate appropriately while maintaining a safe and respectful environment.
Responsibilities

Provide continuous 1:1 nursing care and supervision in the patient’s home

Monitor blood glucose levels and administer insulin based on ordered parameters

Monitor and document the patient’s condition and changes throughout the shift

Provide support and redirection during episodes of confusion or disorientation

Respond calmly and appropriately to agitation or aggressive behaviors

Maintain patient safety and identify/report significant changes in condition

Assist with activities of daily living and other care needs as appropriate

Complete required documentation in the client’s EMR

Communicate relevant updates or concerns to the supervising home health team

Follow the established plan of care and physician orders
Schedule
We are currently staffing 12-hour shifts:

Day: 8:00 AM–8:00 PM

Night: 8:00 PM–8:00 AM

Candidates may indicate preference for days, nights, or both

Compensation
$33–$35/hour, depending on experience and availability.
Qualifications

Active, unrestricted Texas LVN license
-
Current BLS/CPR
-
Private-duty, home health, geriatric, memory-care, or behavioral-health experience strongly preferred

Experience with diabetes management and insulin administration

Comfortable caring for patients experiencing dementia-related confusion, agitation, or behavioral changes

Strong observation, documentation, and communication skills

Dependable and comfortable working independently in a private-home setting
Important Case Information
This position is in a private residence in New Braunfels, TX 78132. There are five cats in the home, so applicants must be comfortable working around cats.
The patient’s spouse is currently away from the home for an extended period, making reliable nursing coverage especially important. The patient benefits from a caregiver who can balance clinical competency with patience, reassurance, and respectful redirection.
