Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) Point Click Care.
- Administer medications and injections accurately following prescribed protocols while maintaining compliance with HIPAA regulations and infection control standards.
- Assist with medical procedures such as IV infusions, catheterization, wound care, tube feeding, and sterile dressing changes utilizing aseptic techniques.
- Support patients with activities of daily living (ADLs), including mobility assistance with Hoyer lifts, personal hygiene, and nutrition management.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with complex needs like dementia, Alzheimer's disease, or developmental disabilities.
- Document all patient interactions thoroughly in EMR/EHR systems like Athenahealth or eClinicalWorks, ensuring accuracy for medical records and billing purposes.
Qualifications
- Valid CNA certification or LPN license in good standing; additional certifications such as Basic Life Support (BLS) are required.
- Demonstrated experience working 6 mos or more in Skill Nursing setting.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology to support accurate patient assessment and documentation.
- Ability to work effectively with individuals with disabilities or cognitive impairments such as dementia or memory care needs.
