Requirements of the Nurse Case Manager:
- MD or compact Registered Nurse
- Discharge planning experience in Hospital setting required
- EPIC EMR experience
Pay & Perks of Registered Nurse Case Manager:
- Registered Nurse Case Manager Pay: $45.50 per hour
- Good work/life balance: Only 40 hours per week.
- 12 week assignments give you the flexibility to see if the company is a good fit.
- Possible temporary to permanent.
- Shifts for Registered Nurse Case Manager: M-F, 8a-4:30p
RN Case Manager Position Objective:
- Coordinates care for a select caseload of patients from triage, care coordination and safe transfer to home. Proactively communicates with payors to ensure approval of treatment plan and required resources. - Maintains knowledge of and responds to clinical system, and fiscal data related to specific patient populations. Collaborates with the multi-disciplinary team, patient/ patient family to ensure effective and efficient transition planning. Coordinates transition to post-acute settings including but not limited to nursing facilities, acute inpatient rehab and/or local shelters. Manages patients based on departmental procedures and maintaining clinical safety and quality. Enters and maintains patient information in all departmental systems, per operational guidelines.
- Must have a current/active CPR card.
