Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate healthcare team. The ideal candidate will possess a strong commitment to patient care, excellent clinical skills, and experience working with diverse patient populations. In this role, you will deliver essential nursing services in accordance with established protocols, ensuring the safety, comfort, and well-being of our residents and patients.
Responsibilities
- Provide direct patient care by administering medications, treatments, and injections accurately and safely.
- Conduct patient assessments, monitor vital signs, and document clinical observations in compliance with healthcare regulations.
- Assist with activities of daily living (ADLs), including hygiene, mobility support, and nutritional assistance.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient’s needs.
- Maintain accurate medical records using electronic health record (EHR) systems while ensuring compliance with HIPAA and confidentiality policies.
- Support infection control practices and ensure aseptic techniques are followed during procedures such as catheter care or specimen collection.
- Participate in discharge planning, patient education, and family communication to promote ongoing health management.
Skills
- Valid licensure as a Licensed Practical Nurse (LPN) in accordance with state regulations.
- Experience working in long-term care facilities, skilled nursing facilities, or assisted living environments.
- Proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for medical documentation.
- Knowledge of medical terminology, anatomy, physiology, and pharmacology relevant to patient care.
- Ability to perform clinical procedures including IV infusion, venipuncture, catheterization, and basic life support (BLS).
- Critical care experience or background in emergency medicine is advantageous for handling acute situations effectively.
- Strong communication skills combined with a compassionate approach to working with geriatric patients and individuals with disabilities or dementia-related conditions such as Alzheimer’s disease or memory care needs.
