Location: St. Johnsbury, VT
Type: Contract | 13 Weeks
Shift & Schedule: Days | Monday–Friday | Possible Weekend Rotation | Guaranteed 36 Hours/Week
Position Overview:
Navitas Healthcare, LLC is seeking Home Health Registered Nurse (RN) for an exciting Travel or Local job in St. Johnsbury, VT.
Responsibilities
- Perform patient admissions, evaluations, and care plan development
- Deliver skilled nursing care in a home health setting
- Collaborate with interdisciplinary healthcare teams and paraprofessionals
- Support coordinated, patient-focused care within the community
- Maintain accurate documentation using Homecare Homebase (HCHB)
Qualifications
- Active Vermont RN License required
- Minimum 1 year of clinical nursing experience preferred
- Current BLS certification required
- Home Health experience preferred
- Experience with Homecare Homebase (HCHB) is a plus
