Responsibilities
- Deliver comprehensive personal care assistance, including bathing, grooming, dressing, and mobility support
- Assist with Activities of Daily Living (ADLs) such as eating, toileting, and transferring using patient transfer methods or equipment like Hoyer lifts
- Monitor and document patient health status by taking vital signs and observing patient conditions
- Administer medications and perform catheter care following medical instructions and home healthcare regulations
- Maintain accurate medical records and documentation using Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems
- Support patients with meal preparation, light housekeeping, laundry, and cleaning to ensure a safe and comfortable environment
- Communicate effectively with patients, families, and healthcare teams to coordinate care plans and update on patient progress
- Follow HIPAA guidelines to maintain patient confidentiality at all times while providing compassionate care
Qualifications
- Prior experience working with individuals with developmental disabilities, dementia care (including Alzheimer's), or in hospice/end-of-life settings is highly desirable
- Demonstrated knowledge of medical terminology, medication administration, vital signs monitoring, and basic life support (BLS) or CPR certification
- Experience working in assisted living facilities, nursing homes, or home healthcare agencies preferred
- Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts or gait belts
- Strong communication skills for effective client interaction and documentation accuracy
- Familiarity with home health regulations and compliance standards such as HIPAA is essential
- Valid driver’s license and reliable transportation are often required for community-based visits
