We are looking for a strong Clinical Manager (RN) to support our growing home health team. This role is hands-on and ideal for someone who enjoys leading nurses, staying clinically involved, and making a real impact on patient care and team success.
This is not a sit-behind-a-desk role — you’ll be actively supporting your team in the field, reviewing clinical work, and helping set the tone for quality care.
What you’ll be doing:
Oversee and support a team of RNs in the field
Provide clinical review of documentation, OASIS, and overall patient care plans
Conduct client/patient visits as needed for quality and support
Assist with onboarding and orientation of new nursing staff
Be a go-to clinical resource for your team
Partner with leadership to improve processes, outcomes, and team performance
What we’re looking for:
Active RN license (CT)
Bachelor of Science in Nursing (BSN) – required
Home health experience (strongly preferred)
Experience with OASIS and clinical documentation review
Prior leadership or mentorship experience (formal or informal)
Someone who is supportive, responsive, and not afraid to jump in when needed
