Real Care, Inc is a Service Provider Agency for adults with developmental disabilities is seeking a full time (40 hours per week) Program Support Coordinator.
Experience in developmental disabilities (areas listed) is preferred:
- Conduct routine file and home monitoring to ensure health and safety of individual receiving services
- Development, writing and implementation of ISPs / BISSPs and Assessments
- Development and submission of HRC packets (psych. medications, rights suspensions, etc.)
- Assisting providers and caregivers in accessing online training resources.
- Monitoring overall compliance documents of providers / caregivers to meet state regulations
- Maintain regular communication with providers and caregivers regarding compliance with training and required documentation.
- Development of Informed Consents
- Participation in Annual Service Plan meetings and IDT meetings
- Ability to commute all through Colorado (with milage reimbursement)
Individual will work closely with Executive Director and Managers.
Individual must be able to perform the following tasks / have skills in the following areas:
- Excellent verbal and written communication skills
- Organized / detail oriented
- Proficient in Microsoft Office Programs (Word, Excel, Publisher)
- Proficient in use of Internet, email and office equipment (printer, fax, copier, etc.)
- Excellent Time Management Skills / able to meet multiple deadlines
- Personable and positive customer service skills (will be backup phone support for receptionist)
- Ability to multi task
- Able to work in busy office setting
- Driving highways
Pay dependent upon qualifications
Individual will be required to have:
- Valid Colorado Driver's license
- Reliable transportation (available throughout the day for attending monitoring(s) and / or meeting(s) -- registered & insured)
- Clean Motor Vehicle Record
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
