Registered Nurse (RN) – CCM/RPM Program Development & Clinical Operations
Rural Oklahoma | 1099 Independent Contractor | Primarily Remote + PRN Field Visits
We are seeking a highly motivated, critical-thinking Registered Nurse (RN) to join the development and implementation of an innovative Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) program serving rural Oklahoma.
This is not simply a traditional nursing position. The RN will be an integral member of the program design and implementation team, helping us build, refine, and continuously improve clinical workflows, the technology platform, patient experience, and day-to-day operations.
We are looking for someone who enjoys solving problems, working with technology, developing new processes, and helping build a healthcare program from the ground up.
Key ResponsibilitiesProgram Design & Implementation
- Participate actively in the design, implementation, testing, and continuous improvement of the Rural Oklahoma CCM/RPM program.
- Provide a nursing perspective during development of clinical workflows, protocols, documentation processes, and patient engagement strategies.
- Work closely with leadership and the technology/platform team to identify workflow gaps and recommend practical improvements.
- Participate in platform testing and implementation, identifying usability issues and opportunities to improve efficiency.
- Help develop processes that are practical for nurses, providers, patients, and rural healthcare partners.
- Assist with development of clinical protocols, escalation pathways, care-management workflows, and quality-improvement initiatives.
- Provide ongoing feedback as the program grows and evolves.
CCM & Remote Patient Monitoring
- Conduct telephonic and virtual patient outreach.
- Review remote monitoring data including blood pressure, blood glucose, weight, and other assigned health metrics.
- Recognize abnormal trends and escalate clinical concerns appropriately.
- Support patients with chronic conditions such as hypertension, diabetes, CHF, COPD, CKD, and other chronic illnesses.
- Provide patient education and reinforce treatment plans.
- Assist patients with remote monitoring technology.
- Coordinate care with providers, facilities, and other members of the healthcare team.
- Maintain accurate and timely clinical documentation.
EMR & Technology
- Must be comfortable working with Electronic Medical Records (EMRs) and healthcare technology.
- Willing and able to learn new software, monitoring platforms, and digital healthcare tools.
- Help evaluate and improve EMR and platform workflows.
- Participate in testing new platform features and provide feedback to improve clinical usability and efficiency.
Rural Oklahoma Field Support
This position is primarily remote; however, the RN must be available for PRN travel to rural Oklahoma facilities when an in-person presence is needed.
Occasional responsibilities may include facility visits, program implementation and onboarding, staff or patient education, troubleshooting workflow or technology issues, and supporting special program initiatives.
Required Qualifications
- Active, unrestricted Registered Nurse license in the State of Oklahoma — REQUIRED
- Graduate of an accredited nursing program.
- Strong clinical judgment and critical-thinking/problem-solving skills.
- Familiarity and experience working with an EMR.
- Comfortable learning and adopting new technology.
- Excellent written and verbal communication skills.
- Strong organizational and documentation skills.
- Ability to work independently and take initiative.
- Comfortable working in an evolving program where processes will be continuously developed and improved.
- Ability and willingness to travel PRN to rural Oklahoma facilities when needed.
- Reliable transportation for occasional field visits.
Preferred Experience
Experience in one or more of the following is highly desirable:
- ICU/Critical Care nursing experience is a significant added bonus
- Chronic Care Management (CCM)
- Remote Patient Monitoring (RPM)
- Care management or care coordination
- Home health or primary care
- Population health
- Medicare patient populations
- Rural healthcare
- Healthcare technology implementation
- EMR implementation or optimization
- Quality improvement or clinical program development
