As a Case Manager, you'll play an important role in helping our clients identify their goals, navigate challenges, connect with resources, and develop the skills and supports they need to thrive. This is a relationship-driven position where your work can have a direct and lasting impact on the people we serve.
Turning Point is a community-based behavioral health organization serving diverse communities throughout metropolitan Chicago. We provide outpatient therapy, case management, residential services, groups, and other behavioral health support. Turning Point has consistently earned the highest rating from the Commission on Accreditation of Rehabilitation Facilities (CARF).
About the Role
Our Case Managers work with adults to help them maintain or gain independence and achieve a higher level of functioning. You'll build supportive relationships with clients while helping them navigate both everyday challenges and larger life goals.
No two days are exactly alike. Your work may include helping a client apply for benefits, connecting someone with a healthcare provider, developing a monthly budget, supporting independent living skills, or helping a client maintain their housing. You'll also provide support in the community and collaborate with other members of the treatment team.
What You'll Do:
- Build trusting, supportive relationships with clients while maintaining appropriate professional boundaries.
- Assess clients' individual case management needs and develop individualized treatment plans.
- Help clients identify and work toward personal goals related to independence, stability, wellness, and community living.
- Assist clients in obtaining public benefits, entitlements, and other available resources.
- Connect clients with healthcare and behavioral health providers, including therapists, psychiatrists, dentists, primary care providers, and other community services.
- Help clients develop and manage monthly budgets and strengthen financial skills.
- Support clients in maintaining their apartments and developing independent living skills.
- Provide client support in community settings and help clients navigate community resources.
- Facilitate skills-based groups through our PSR program.
- Provide substance use-related services as part of the client's overall plan of care.
- Maintain timely and accurate documentation of services.
- Collaborate with colleagues and other providers to support clients' overall well-being and progress.
Required Qualifications
- Bachelor's degree in Human Services, Social Sciences, or a related field.
- Valid driver's license and current vehicle insurance.
- Strong oral and written communication skills.
- Ability to maintain professional relationships and model appropriate professional behavior.
- Ability to work with a diverse population and approach clients with respect, dignity, and empathy.
Preferred Qualifications
- Working knowledge of IDHS entitlements and benefits.
- Familiarity with community-based resources and social service systems.
- Experience working with individuals experiencing mental illness, substance use concerns, housing instability, or other barriers to independence.
- Experience in community behavioral health, case management, social services, or a related field.
