All-4-One Home Healthcare Services, Inc. has an opening for a Quality Assurance Coordinator. Candidates must have home health experience to include knowledge of meeting visit frequency under MD orders as relates to approved Plan of care and monitoring Plan of Cares submission and must be able to thrive in a busy and fast-paced office environment.
Responsibilities will include monitoring of scheduling coordination of disciplines ordered for client services and QA of clinical notes to ensure meeting care plan requirements for completion of visits and needed orders and missed visits. Will assist with reviewing the plan of treatment and the medical plan for patients receiving skilled nursing services for the patient admission visit that is generated the initial 485 certification/485 recertification and therapy POC's and Eval forms for submission to physician for signature by discipline . Will scan all patient documents into medical home care system.
Will track and monitor submission of certification plans and will also visit MD offices and/or facility locations to drop off/pick up Certification Care Plans as required. Follow up with MD or Facility for required documentation.
Must be detail oriented, hard working, well organized and professional and able to multi-task, problem solve and the ability to prioritize tasks.
