Kingly Home Health is seeking a compassionate and skilled Physical Therapist (PRN) to join our growing home health team. This role is ideal for a licensed Physical Therapist with prior home health experience who is looking for flexible, part-time PRN work while making a meaningful impact in patients’ lives.
Pay Rate of $60-$75 per visit
Our Physical Therapists provide high-quality, patient-centered care in the comfort of patients’ homes, supporting functional mobility, independence, and overall quality of life.
Key Responsibilities
- Perform comprehensive physical therapy evaluations in the home setting
- Develop and implement individualized treatment plans based on patient needs and physician orders
- Provide therapeutic exercises, gait training, balance training, and functional mobility interventions
- Educate patients and caregivers on home exercise programs, safety, and fall prevention
- Accurately complete all documentation in a timely manner in compliance with CMS and agency standards
- Communicate effectively with physicians, interdisciplinary team members, patients, and families
- Participate in care coordination to ensure continuity and quality of care
Qualifications
- Current Physical Therapist license in the state of practice
- Minimum of 1 year of experience, preferably in home health
- Strong clinical assessment and critical-thinking skills
- Excellent communication, documentation, and time-management abilities
- Ability to work independently while collaborating with an interdisciplinary team
- Valid driver’s license, reliable transportation, and current auto insurance
