What You'll Do
- Referral Development Build and maintain a robust referral pipeline through hospital discharge planners at Hoag and Mission Hospital, skilled nursing facilities, assisted living communities, physicians, and geriatric care managers across coastal Orange County — consistently generating qualified client starts and referral source growth.
- Relationship Management Turn cold contacts into long-term referral partners through consistent follow-up, community presence, and trust built over time.
- Strategic Planning Develop and execute a data-driven 90-day territory growth plan. Track activity, measure results, and adjust strategy based on what's working.
- Family Intake Support Assist with initial family consultations and help ensure warm, confident starts of care.
- Community Presence Represent Home Matters at local senior expos, health fairs, and community events throughout Newport Beach, Huntington Beach, Costa Mesa, Laguna Beach, Dana Point, and surrounding coastal communities.
Who You Are
- 2+ years in Home Care sales, marketing, or business development (private duty/non-medical — non-negotiable)
- Deep understanding of the California referral landscape, payer dynamics, and private-pay home care
- Proven track record of territory growth and consistent referral generation in a competitive market
- Existing relationships with OC hospital discharge planners, SNFs, or assisted living communities is a major plus
- Self-starter who takes initiative, builds momentum, and consistently follows through without requiring constant direction.
- Highly organized with strong follow-through and consistent CRM documentation.
- Empathetic and persuasive — you build trust quickly with healthcare professionals and families alike
- Comfortable with CRM platforms (Salesforce, HubSpot, or similar) and Microsoft Office
- Polished, professional, and reliable in every interaction
- Valid CA driver's license and reliable transportation required
Compensation & Benefits
- Base salary
- Uncapped performance bonus opportunities tied directly to retained client starts, referral source development, and territory growth
- Client start bonus — tiered by weekly hours, paid after 30 consecutive days of active care
- Referral source growth bonus — earned as new referral sources generate billed revenue milestones
- Quarterly territory growth bonus — based on total retained client starts per quarter
