Job Summary
We are seeking compassionate and dedicated Home Health Aides and Certified Nursing Assistants (CNAs) to provide exceptional in-home care to our clients. Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report any abnormalities promptly
- Administer medications accurately following medical instructions and document medication administration in medical records or electronic health record (EHR) systems
- Support patients with specialized needs such as catheter care, dementia care, Alzheimer's care, memory care, hospice care, and end-of-life support
- Use proper techniques for patient transfers and mobility assistance, including Hoyer lifts and other assistive devices
- Maintain compliance with home healthcare regulations and HIPAA privacy standards while managing sensitive patient information
- Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe environment for patients
- Document all patient interactions thoroughly using medical documentation protocols and electronic systems to ensure continuity of care
Requirements
- Proven experience working as a Home Health Aide or CNA in settings such as assisted living facilities, nursing homes, or home healthcare agencies
- Knowledge of medical terminology and familiarity with EMR/EHR systems for documentation purposes
- Experience working with individuals with developmental disabilities or geriatrics is highly preferred
- Certification as a CNA or equivalent state-approved training program completion required; CPR and First Aid certifications mandatory; Basic Life Support (BLS) certification is a plus
- Ability to lift heavy objects safely and perform physically demanding tasks such as transferring patients using Hoyer lifts or assisting with mobility needs
- Strong patient observation skills to monitor health status effectively and respond appropriately in emergencies
- Excellent communication skills to collaborate with healthcare teams, patients, and families while maintaining HIPAA confidentiality
- Flexibility to work various shifts including live-in arrangements or extended hours as needed
