Responsibilities
- Deliver direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status by recording vital signs and reporting any changes to healthcare professionals promptly
- Assist with medication administration following prescribed protocols and maintaining accurate medical documentation in Electronic Health Record (EHR) systems
- Support clients with specialized needs including Alzheimer's care, dementia care, hospice care, and working with individuals with developmental disabilities or memory impairments
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices to ensure safe patient handling and mobility
- Maintain a clean and safe environment by performing light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control standards are met in accordance with HIPAA regulations
- Document all patient interactions accurately within medical records while respecting patient confidentiality
Qualifications
- Proven experience as a Home Health Aide (HHA), nursing assistant, or in a similar caregiving role within home health or long-term care settings
- Knowledge of medical terminology, vital signs monitoring, medication administration, and basic nursing skills such as catheter care and CPR certification
- Experience working with seniors, people with disabilities, or those requiring memory or dementia care is highly desirable
- Familiarity with EMR/EHR systems for documentation and medical records management
- Ability to work independently while following healthcare plans and protocols diligently
- Compassionate attitude combined with excellent communication skills to connect effectively with clients and their families
- Valid HHA certification or equivalent state licensing; CPR certification required
