Compensation
- Start of Care (SOC): $110 – $125 per visit
- Routine Visits (Follow-ups): $55 – $65 per visit

Key Responsibilities
- Complete Start of Care (SOC) assessments and nursing evaluations
- Provide skilled nursing visits per physician orders
- Develop and update plans of care (POC)
- Monitor patient condition and report changes promptly
- Administer medications and provide patient education
- Educate patients and caregivers on disease management and home care plans
- Complete OASIS documentation and maintain accurate EMR records
- Ensure compliance with Medicare home health guidelines
Required Qualifications
- Active Arizona Registered Nurse (RN) license
- Current CPR/BLS certification
- Minimum 1 year nursing experience
- Strong clinical, documentation, and communication skills
- Reliable transportation for home visits
Preferred Qualifications
- Home Health RN experience
- Experience with OASIS and EMR systems
- Background in geriatrics or chronic care
- Bilingual candidates encouraged to apply
