JOB SUMMARY
The Health Services Navigator works in close collaboration with a clinical and administrative supervisor on a variety of activities that support improving patient outcomes and care coordination. This position requires empathy, compassion, and an understanding of the Ambulatory Health Services (AHS) patient population.
JOB SPECIFICATIONS

Obtain patient records from outside providers (e.g., discharge summaries, consult reports, diagnostic results, etc.).

Follow up on patient referrals and diagnostic orders (e.g., track appointments and orders to completion, obtain outstanding reports, etc.).

Provide care coordination services to at-risk patients as needed (e.g., assist patients with outpatient mental health referrals schedule intake appointments, etc.).

Perform outreach to patients due for care (e.g., at-risk patients with no upcoming appointment, patients due for screenings, patients discharged from emergency department/hospital, etc.).

Provide patient education (e.g., how to make an appointment, how to contact the HC after hours, proper technique for remote patient monitoring, etc.).

Refer patients to internal and external resources, as indicated.

Track and document the outcome of activities in logs and provide monthly statistics.

Monitor phone calls/voicemails and written communication and respond to inquiries in a timely manner.

Education
-
High School Diploma or equivalent required.

Some college or bachelor’s degree preferred.
Skills/Experience
-
Must have excellent customer service skills.

Prior experience in providing direct services in a healthcare or human/social services organization preferred.

Must have knowledge of basic computer usage, including familiarity with the Microsoft Office Suite, and the ability to learn and use new technology and software.

Prior experience with electronic health records preferred.

Must have the ability to approach and interact with patients, including interviewing patients and engaging them in appropriate services.

Must have the ability to track activities completed and services provided.

Knowledge of case management procedures preferred.

Must have the ability to interact professionally with AHS staff.

Prior experience with external stakeholders preferred.
