We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) Caregiver to join our dynamic healthcare team. In this vital role, you will provide high-quality, personalized care to patients across various settings, including long-term care facilities, assisted living communities, and home care environments. Duties
- Deliver comprehensive patient care by monitoring vital signs, administering medications, and managing treatments such as IV infusions and catheter care
- Assist with activities of daily living (ADLs), including mobility support, hygiene, and nutrition management
- Conduct thorough patient assessments and document clinical observations accurately using Electronic Health Record (EHR) systems
- Support specialized care needs such as dementia, Alzheimer’s, memory care, and behavioral health interventions
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet regulatory standards and patient goals
- Provide emergency response when necessary, including first aid, airway management, and acute pain management
- Ensure compliance with HIPAA regulations and maintain strict confidentiality of all patient information
Skills
- Proven experience working with individuals with developmental disabilities, dementia, or in memory care settings
- Strong knowledge of medical terminology, physiology, and clinical procedures such as venipuncture, injections, and sterile technique
- Familiarity with EMR/EHR systems like Epic, Cerner or Athenahealth for accurate documentation
- Skilled in patient assessment, triage, and diagnostic evaluation within various healthcare environments including hospitals, nursing homes, or outpatient clinics
- Ability to perform complex tasks such as tube feeding, catheterization, ventilator management, and wound suturing when required
- Experience in acute care settings such as ICU or Level I Trauma Centers is highly desirable
- Excellent communication skills for caregiver-patient interactions and interdisciplinary collaboration
- Knowledge of long-term care regulations and best practices for senior care and hospice services
